Maybe there's a difference between fuck-up and error, though? It's been a "squishy" part of understanding safety, incidents, performance, efficiency, etc. for a long time. Mostly, Ron just likes saying "squishy." So, what is human error? Is it a legitimate concern or a red herring? Where does intent and context come into the equation?
